DOT SAP Evaluation After a Violation
After a violation, the U.S. Department of Transportation’s 2009 (DOT) guidelines require that all employees in DOT safety-sensitive duties undergo an evaluation by an accredited SAP professional.
I am a certified DOT Substance Abuse Professional (2018 Certification SAP) with over 30 years of post-master’s clinical experience. My SAP Evaluation includes:

- Treatment
- Follow-up testing

SAP Credentials
I am a Licensed SAP Professional having successfully completed the required SAP professional hours and examination. I am also a Licensed Clinical Social Worker and a Certified Drug and Alcohol Consultant.
A Licensed SAP Professional must (one of below):
- Licensed physician (Doctor of Medicine or Osteopathy)
- Licensed or certified social worker
- Licensed or certified psychologist
- Licensed or certified employee assistance professional
- State-licensed or certified marriage and family therapist
- Alcohol and drug abuse counselor certified by the National
Association of Alcoholism and Drug Abuse Counselors Certification Commission (NAADAC), or by the International Certification Reciprocity Consortium/Alcohol and Other Drug Abuse (ICRC), or by the National Board of Certified Counselors, Inc. and Affiliates/Master Addictions Counselor (NBCC).
Knowledge Required
I have clinical experience in the diagnosis and treatment of substance abuse-related disorders. I also understand current DOT agency regulations and SAP guidelines.
SAP Professional Training
As a SAP Professional, I have received the prerequisite qualification training.
Examination
I have completed training and passed the SAP examination given by an accredited SAP testing organization.
Continuing Education Required
I have completed the required 12 professional development hours (e.g., Continuing Education Units) relevant to performing SAP duties.
